Your results and treatment plan will be sent to you directly via email. If requested and if applicable, a copy of your prescription can also be sent to you.
If you choose, you may share those documents with any of your health care providers. If you would like us to send your records to anyone other than yourself, we would require you to complete an Authorization for Release of Medical Information form prior to us sending your records.
